Welcome to the private psychiatric practice of A. Kenneth Fuller, M.D. Our goal is to provide you timely, respectful, quality service in a pleasant practice environment. In order to serve you more efficiently, please read the following policy closely.
HOURS: Our office assistant is generally present after 9:00 a.m. Monday through Thursday, except holidays. CLOSED FRIDAYS.
APPOINTMENTS: The office assistant schedules appointments. Dr. Fuller usually sees patients from 9:00 a.m. to 5:00 p.m. Each session is either 25 minutes or 50 minutes in duration. We do not overbook or double book; the time you schedule is yours. If you cannot keep your appointment, please let us know 24 hours in advance to avoid a charge. The frequency of appointments is very individualized, because each person and his or her needs are different. Some need to come in more frequently than others.
PAYMENT: Full payment is due when you arrive for your appointment. Please hand your payment to the office assistant. Personal checks, cash, money orders, Visa and MasterCard are accepted. If you cannot afford services offered by this practice, mental health services are offered at your local mental health center with fees based on your ability to pay.
INSURANCE: Dr. Fuller is not a provider for any insurance companies. Your insurance policy is a contract between you and your insurer. You are responsible for all charges incurred, as well as for any services that are not covered by your policy, such as telephone consultations, missed appointments and reports. As a courtesy we file insurance claims.
EMERGENCIES: If you have an immediate life and death emergency, call 911 or go promptly to an Emergency Room or Urgent Care for assistance. During office hours, Dr. Fuller will return calls as possible. He makes a strong effort to return all patient calls on the day received. Please give the office assistant a summary of your need or concern.
AFTER HOURS, WEEKENDS, and HOLIDAYS: When the office is closed, a brief message can be left on the confidential answering machine. If you have an immediate life and death emergency, call 911 or go promptly to an Emergency Room or Urgent Care for assistance.
PRESCRIPTION REFILLS: Dr. Fuller's routine practice is to write prescriptions to cover your needs until your next appointment. There should be no need for additional refills if you keep scheduled appointments. If an exception occurs, please call during office hours at least two working days before you will run out. Some controlled substances (i.e., Ritalin, Adderall, Dexedrine) cannot be refilled by phone and require a written prescription. Refill requests are not emergencies. Dr. Fuller will authorize refills during business hours only.
State and federal law mandates that prescriptions and refills only be approved for active patients. Persons not seen in the past four months are not considered active or current patients. Refills will only be approved for current patients in good standing who have scheduled follow-up appointments. Medication changes generally require appointments so they can be adequately considered, explained, and discussed.
DOCTOR-PATIENT RELATIONSHIP: Dr. Fuller becomes your psychiatrist when a mutual agreement is made to work together after the initial evaluation is completed. This relationship is a professional, cooperative partnership in which we both have responsibilities to work toward agreed upon goals. Because of the nature of psychiatric treatment, you must be seen at least every three months to be considered an active or current patient. The doctor-patient relationship is terminated on the 120th day following the date of the last appointment you attended. No additional notice is required for this termination of the doctor-patient relationship to occur.
RECORDS: There is a charge to copy records. A completed, signed release of information is required.
MISSED APPOINTMENTS: You will be charged the full fee for appointments missed, and canceled or changed with less than 24 hours of notice.
REPORTS, FORMS, PHONE CALLS, and LETTERS: The charge for phone calls to patients, letters, reports, medication authorizations, or calls done on your behalf to other clinicians or insurance companies/agencies is $225.00 per hour. Your account must be paid in full before Dr. Fuller completes forms, reports or letters.
SOLE PROPRIETOR: Dr. Fuller's practice is fully independent of all persons and agencies.
HIPAA: Dr. Fuller's practice meets or exceeds HIPAA privacy standards.
